Privacy and Sharing of Information
I authorize the clinic and its associated health professionals to collect my personal and medical information as documented above. In addition, I authorize the clinic and its associated health professionals to communicate with my family doctor and/or referring doctor as deemed necessary for my beneficial treatment. I also understand that my personal and medical information is confidential and will only be disclosed to third parties with my permission.

Parking Policy:
In Home Chiro asks that patients make parking as easy as possible for the doctor. We require that the patient leaves their parking spot open for the doctor. Patients do not need to leave their parking spot open if parking is available within half a block. We allow for 5 minutes to park and arrive at your door. If parking takes longer than 5 minutes, then that time will be taken out of your treatment time. This policy helps ensure that each patient gets their scheduled appointment time.

Cancellation Policy:
Your appointment time is reserved just for you. Late cancellation or missed visit leaves a hole in the doctors’ day that another patient could have filled. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a $55 cancellation fee.

Space Requirement:
In Home Chiro requires a space of at least 4 feet wide and 7 feet long to set up safely. We also require that the working space is cleared of clutter and furniture. Any time that the doctor has to spend clearing the space will be taken out of the treatment time.

Pet Policy:
In Home Chiro loves all pets and is excited to meet your furry family member. In order to help the doctors please do not allow your pet to jump on the doctor or any equipment. If the pet causes any dirtying or damaging of clothing will result in a minimum of a $50 charge. Any dirtying or damaging of equipment will result in a minimum $150 charge. This will be paid immediately.

Payment Policy:
In Home Chiro takes cash, checks, HSA, FSA, Debit cards, Credit cards, and all major card providers. We do not accept insurance, medicare, medical, workers compensation, personal injury cases. Unfortunately, anyone who has medicare cannot book an appointment with one of our chiropractors. Extra fees are charged for weekends and for appointments after 7pm.

Clothing Policy:
In Home Chiro suggests wearing athletic attire such as: Shorts, yoga pants, tank top, T-shirt, ect. During a normal chiropractic visit, the patient and doctor work together to find the best treatment possible. These treatments often involve stretches, and the patient should wear clothing that they feel comfortable stretching in.
